  9. HEP B/Immunisations Report/ Document Evidence of Immunity against Hep B – Antibodies (Titre Levels)

  10. Relevant Degree/Masters

  11. Medical Performers List letter of approval (can be retrieved online NHS England)

  12. Certificate of Completion of Training (CCT/JCPTGP) Certificate

  13. Basic Life Support/Advanced Life Support Certificate
